    Parent Letter

    Working together between home and school helps us to ensure that your child is safe and that their individual needs are met.

    To ensure that all medical information we hold for your child is current and accurate, we ask parents and carers to complete the relevant forms in line with the Department for Education guidance “Supporting Pupils with Medical Conditions” (December 2015).

    If your child has a medical condition, please complete a Care Plan outlining their medical needs and any emergency action that may be required.

    If your child requires medication during the school day that has been prescribed by a doctor, please ensure the appropriate form is completed.
